Employees
departing the National Weather Service are required to complete a CD-126 as
part of the exit clearance process. Improper processing of clearance
information can lead to delays in receiving a final pay check and/or
indebtedness to the Government.
Attached is a sample CD-126 for
your staff to use as a guide. Complete all blocks on the front with
the exception of Imprest Funds (no longer required). Note, in addition
to the two blocks on the back requiring attention, a third block, “Computer
Access,” is checked. The completed W/CFO31 User
Account Clearance Form (attached) must accompany the CD-126 to
meet Computer Access clearance requirements for signature.
Employees with a Citibank Government Travel Card should deliver the
card to Beth Downs when requesting her signature on the CD-126 along
with the Government Purchase Travel Card Maintenance Form located on
the website below under Travel Card Forms:

Please allow ample time before separation to clear the form. Increased
work loads and Alternate Work Schedule days need to be taken into consideration
and the process initiated before the last work day. Only blocks 1,
2, and the Security block on the back should remain blank until the
final day. Bring the form to W/CFO3, Betty Dodds, 18313. In her absence
Nicholas Leivers or Nicholas Scheller will sign off and retain the
employee’s building pass and any outstanding keys or key cards.
