The National Weather Service at Caribou, Maine has a brand new and very easy to use storm reporting form.  You start off by selecting the type of weather to report and hit the "next" button.  You then provide information on the time you observed the weather, and any details.  For example, if there was wind damage you could give more specifics on what was damaged, i.e. several 1 foot diameter trees were knocked down by the high wind.  Next, we will need to know the location of your report.  We do not store it permanently, only as a latitude and longitude for the report.  You can automatically use your device's location or manually enter the address information in the box, or you can zoom into you location on the map.  Finally, if you wish to provide a name, spotter ID, or other contact information you can do so, but this is optional.  https://inws.ncep.noaa.gov/report/
