Registration Form Download

  1. Old spotters (previously trained)
    1. Old Spotters must check in with the registration table at the spotter talk.  Spotter registration forms will be available at the table,
      • Find name on registered spotter sheets.  Names are listed alphabetically by county of residence.
      • Ensure information is correct (especially email)
      • Check Mark next to the spotter name to record attendance.
      • Changes to Information:
        • Small Changes (phone or email)  Pen and ink the changes on the spotter sheet
        • Large Changes (Address)  Fill out a new spotter registration form.  Clearly mark on the top of the form that the person is an existing spotter with a change of information.  Record spotter number if available.
      • DO NOT assign returning spotters another Spotter Number.  If they have their current number you can place that information in the spotter number box, otherwise leave the spotter number on the registration form blank.
  2. New Spotters
    1. New Spotters must fill out the entire registration form and be assigned a new spotter number.
    2. Assigning spotter numbers
      1. Assign numbers from the NWS CLE Spotter Number spreadsheet
      2. Link to NWS CLE Spotter Numbers
      3. Spotter numbers should be assigned based on the county that the spotter resides.  Do not assign the number based on the location of the training.
      4. Spotter Numbers are assigned in the following format CCC-XXX.  CCC is the county FIP code and XXX is the next sequential spotter number available for the county (NWS CLE Spotter Numbers).  For example, a new spotter in Cuyahoga County might have a number that looks like 035-456.  035 - County FIP code and 456 is the next spotter number available on the spreadsheet.
      5.  NWS CLE Spotter Number spreadsheet crossing out an assigned number:
        1. Click on the number that you assigned the spotter for that county
        2. Press the Strike Through Box on the Menu to put a line through the number to indicate it was assigned (S with a line through it on the menu).
        3. Note:  This is a google document spreadsheet.  Several people can work on the sheet at the same time.  If you see other boxes highlighted with a color, this means another person is also working in the spreadsheet.
      6. Provide the spotter with their spotter number.  You can give the spotter a website to print a card or if you wish to print your own spotter cards, links are provided to business card templates of the spotter card.  Please make sure the spotter writes their number down on something before they leave..

      7. Spotters outside of the National Weather Service Cleveland forecast area.
        1. Spotters that take the training that reside in a county outside of the NWS CLE forecast area may still take the training and will receive credit for the training. However,  we DO NOT assign them an NWS CLE spotter number since they are not in our forecast area.  Follow the procedure below.
          1. Have the spotter fill out the information on the NWS CLE Registration form.
          2. DO NOT assign a spotter number
          3. Place a small note or remark on the top notifying us that it is an out of area spotter.  (i.e.  WFO Wilmington area,  WFO Detroit spotter or Out of Area or something similar is fine).
          4. Return the application to NWS CLE with the other applications.
